INCOMPLETE OR UNSIGNED ENTRY FORMS WILL NOT BE ACCEPTED:

I hereby acknowledge my voluntary participation in the 5K run event. In consideration of my entry being accepted, I intend to be legally bound, and hereby for myself, my heirs and executors waive and release all rights and claims for damages which may hereinafter accrue to me against FLORIDA PUERTO RICAN PARADE, INC. and the sponsors of the race which I am entering. I assume all risks associated with participation in this activity, including but not limited to: falls, contact with other participants, high heat, humidity, rain, traffic, conditions of the route, automobiles, fractures, cuts, and I attest that, if I am pregnant, disabled in any way, or have recently suffered an illness, injury or impairment, I have consulted with a physician before participating in the event.

This release extends to any liability arising out of or in any way connected with medical treatment and transportation provided in the event. it also extends to any subsidiary or political division thereof, their respective officers, agents, directors, representatives, successors, assigns, and sponsors for any and damages or injuries which may be sustained and suffered by me in connection with my association and participation in the FLORIDA PUERTO RICAN PARADE GUA’KIA 5K.

If I should suffer injury or illness I authorize official of the race to use their discretion o have me transported to a medical facility, and I take full responsibility for this action. I certify that I am physically fit and have sufficiently trained for the completion of this event. I hereby grant fullpermission to any and all of the foregoing to use any photographs, videotapes, motion pictures, recordings or any other record of this event for any purpose Bicycles, dogs (unless service required), inline/roller skates are prohibited. The use of headphones is discouraged. Runners and walkers with baby joggers or strollers are required to follow stroller restrictions and check-in on race day.

COVID-19 Security measures: During the event participants and volunteers and staff will be required to maintain a social distancing of 6 feet and the use of a mask at all times except during the physical event. There will be temperature control and sanitation stations
